Ration Card Form UP Online Registration Procedure for Renewal Surrender and Correction

When you submit your ration card registration form you have to wait for few days from 0-30 days minimum in order to get your ration card. Depending on the type of request you made for the rashan card you have to wait accordingly. The waiting period follows as:

1. Duration to get new card after registration - 30 Days.
2. Duration to get correction on it - 15 Days
3. Duration after surrendering in Office - 15 Days

You need to fill application with required information:

1. Name of district
2. Block name
3. Name of tehsil
4. Village name
5. Municipal name
6. Revenue Office Name
7. Head of family name [with father’s / husband / mother’s name / category]
8. Complete address [house number / mohalla / ward / pin code / mobile number]
9. Having electricity connection [yes / no]
10. Having gas connection [yes / no]
11. Name of gas agency and customer number
12. Complete permanent address
13. Duration of living in state
14. Living as home owner / tenant
15. Type of house
16. Complete details of family members [name / gander / relation with head of house / age / monthly income / address business or office
17. Select the option which document you are submitting with form
18. Fill Declaration form

==> Then do your signature (hastaachchar) and attach a photo with application form along with the thumb print (Angunthay ka Nishan)

After the submission of the rashan card form the process verification officer will come at your home address and he will check the all documents and verify it. After that he / she will provide you a date to go and collect your new ration card at the office location he will tell.

If you or any citizen or family member is going to leave a specific area and going to a new district or state then they need to submit the cancelation / surrender / samparan request form 04 that is available to be downloaded at up govt. online site or e-district website.

==> The request application form need to fill as per the below given format:

1. राशन कार्ड प्राप्‍त करने के लिये आवेदक खाद्य आपूर्ति / सी0 एस0 सी0 सेन्‍टर विभाग में पूर्व निर्धारित फार्म डी01 के अनुसार आवेदन  करेगा। 2.  आवेदक अपने परिवार की दो सत्‍यापित फोटो (पासपोर्ट) आवेदन पत्र के साथ प्रस्‍तुत करेगा। 3. आवेदक (उपभोक्ता) एक हलफनामे है कि उसका भारत में कहीं भी पहले और उसके परिवार के सदस्यों के नाम किसी भी राशन कार्ड में शामिल नहीं हैं की घोषणा देगा। आवेदक अपने स्‍थायी निवास तथा पिछले पांच वर्षो के पते की भी घोषणा करेगा। 4. आवेदक एक निश्चित तारिख के साथ एक पर्ची प्राप्‍त करेगा, जिसके अनुसार वह सम्‍बनिधत अधिकारी से सम्‍पर्क कर सकता है। 5.  आवेदक के द्वारा दी गयी सूचना को इंस्‍पेक्‍टर आवेदक के निवास पर जाकर सत्‍यापित करेगा। 6. राशन कार्ड जिला खाद्य और आपूर्ति के कार्यालय से एकत्र किया जा सकता है
